Staff COVID Swabbing Service

Lateral flow registration form

We are now asking all staff to complete a new lateral flow testing registration form. This form must be completed (once only) by all staff using an existing box of lateral flow tests, or a new box.

Please be aware that if you take a box of lateral flow testing kits, you are committing to recording your results twice a week, agreeing to your data being passed to Public Health England.

Form to be filled out each time you test (we have shortened it)

Going forward, you can use our shorter online form to record your lateral flow results. Results must be recorded within 24 hours of taking the test.

Even if you have had the COVID-19 vaccination, it is really important to complete lateral flow testing twice weekly. Any staff member who has received a positive COVID-19 PCR test, should wait 90 days from their positive test, before recommencing lateral flow testing.

This is a great opportunity for all our staff and we are grateful for your participation.

Opting out

If for whatever reason, you do not want to be involved in lateral flow testing, please complete our short survey to let us know why. Or if you have taken a lateral flow testing kit and for whatever reason you want to opt out please contact
