For those patients who have completed their hospital based breast cancer treatments we offer an ‘open access policy’. At a consultation with an Advanced Nurse Practitioner, we provide a treatment summary and in collaboration with the patienta holistic needs assessment is discussed, creating an on-going care plan, signposting patient led initiatives.  A programme of future surveillance is established and we highlight  the signs and symptoms that  should  be  reported providing contact details to ‘Open access follow up’. This way a patient seeks assessment when they have concerns and need support.  It is no longer necessary to wait for regular follow up appointments or be seen when a patient is well and wishes  to ‘move on ‘ with their normal lives.

We aim to provide the knowledge and confidence for patients to recognise and manage symptoms related to their previous diagnosis of breast cancer or side effects of treatment. We have an Open Access arrangement for the patient to report concerns via our telephone service.
