Our team works to improve the quality of life of you and those close to you when you have a very serious or life-threatening illness.
The team includes:
- Medical consultants
- Nurse consultant
- Clinical nurse specialists
- Social worker
- Palliative Care Clinical educator/Advance care planning facilitator
- Advance care planning coordinator
We work closely with all clinical teams, the hospital’s chaplaincy team, cancer counsellors and the acute oncology service.
We can also coordinate referral to local community teams and hospices for ongoing care and support.
When our team is involved in a patient’s care, they will provide you with a contact number which can be used between 09.00 and 17.00 every day.
National Audit Care at the End of Life (NACEL)
Compared to the national average, patients at the end of life admitted to Kingston Hospital and those important to them are receiving a higher standard of care. This includes the quality of communication held with the patient, their families and carers, as well as the implementation of individual plans of care, which consider the needs of those important to them.
